Key Differences
| Feature | Troponin | CK-MB |
|---|---|---|
| Specificity for heart | Very high | Lower — also found in skeletal muscle |
| Sensitivity | Very high, especially high-sensitivity assays | Lower |
| Stays elevated | 7-10 days | 2-3 days |
| Current recommendation | Preferred single test | Largely replaced by troponin |
Why Timing MattersBecause CK-MB clears faster than troponin, it can be useful for detecting a second heart attack shortly after a first one — but for initial diagnosis, troponin is now the standard everywhere.
Is CK-MB still used at all?
Rarely as a standalone test now, but occasionally alongside troponin in specific circumstances like suspected re-infarction shortly after a first heart attack.
